Participant Waiver & Release of Liability

One form per child.

* In consideration of my child’s (the “Participant”) participation in a Father Son Roller Skating, organized by agents of Congregation Beth Jacob, Inc. (“CBJ”), to engage in various activities in, on, and around said CBJ property (the “Property”) and/or at the event facility (the activities on the Property and the event activities collectively referred to herein as the “Activities”), the Undersigned:

Agrees that prior to engaging in any of the described Activities, the parent and/or the Participant shall inspect the Property (and, if applicable, the event equipment used). If the parent and/or the Participant believes anything is unsafe, the parent aAcknowledges and fully understands that the Participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and damage to property which might result not only from his/her actions, inactions, or neReleases, waives, discharges, saves, holds harmless, and covenants not to sue, CBJ, its officers, directors, members, and their heirs, next of kin, assignees, guests, and agents, all of which are herein after referred to as “Releasees”, from any and aThe Participant has prepared himself/herself for participation in the Activities in which he/she chooses to engage. The parent and/or the Participant warrants and represents that the Participant is in good physical health and condition, and that he/she isThis Waiver and Release of Liability shall remain in effect for as long, and as often, as the parent and/or the Participant enters the Property and/or engages in any of the afore-described Activities.
